GROUP COACHING AGREEMENT
 

Thank you for your interest in Group Coaching. By making a payment for this Service, you confirm that you (the “Participant”) accept the conditions set forth herein upon entering into Coaching Services with Quilting Health LLC (the “Coach,” which includes any employees, members, managers, agents and affiliates of Coach).

 

COACHING RELATIONSHIP & RESPONSIBILITIES

 

Coaching is a co-creative professional relationship between the Coach and Participants (the "Parties"). It can be a powerful experience rooted in mutual trust and respect, that extends the Participants the opportunity to increase self-awareness, develop goals and valuable new ideas, as well as taking action toward these. Coaching topics focus on something that is current, unresolved, and important to the Participant. Coaching involves facilitated discovery, with a conversation focused on the present and creating the future you want by accessing your creative, resourceful, and whole self to make more resonant choices.

Group Coaching provides a collaborative space for personal growth and development, with confidentiality and trust at the foundation of every session. Participants receive professional coaching, enriched by the collective power of peer support, insights, and accountability.​ Coaching does not involve the diagnosis or treatment of mental disorders as defined by the American Psychiatric Association. Coaching is not a substitute for therapy or mental health care and is distinct from mentoring, advising, or facilitation.​

By enrolling in Group Coaching, you commit to curiosity, courage to trust and share, bringing a topic to the session, being present and prompt for the session, as well as being open to discoveries, challenges, and action steps by applying new awareness and behavior. You are responsible for timely relevant communication with the Coach and scheduling-related action. You accept and agree that you are 100% responsible for your progress and results. 

 

The Coach is responsible for determining the approach to coaching sessions, shall maintain the ethics and standards of behavior set by the International Coaching Federation (ICF) and will provide direction to Participants in compliance with ICF ethics and standards.

 

All Parties agree to be transparent and respectful toward one another during the sessions, to share openly and straightforwardly.

CONFIDENTIALITY

 

Coaching is a confidential relationship. All Parties agree not to make any audio or video recordings of sessions. The Parties agree not to disclose, reveal or make use of any confidential information shared during discussions or otherwise, except as may be required by law.​ Confidential information shall mean any information disclosed by a Participant or Coach as part of Group Coaching that is marked as confidential or that should reasonably be understood to be confidential such as, but not limited to, personal disclosures, ideas or information that is not publicly available.

 

The Coach engages in training and continuing education pursuing and/or maintaining ICF credentials. This process involves possible verification by the ICF. The Participant agrees to have their name, contact information and start and end dates of coaching, including hours and meeting dates, shared with ICF staff members and/or other parties involved in this process for the sole and necessary purpose of verifying the coaching relationship. No personal notes will be shared.

The Coach will maintain documents, information and data acquired or shared during group coaching in a format of the Coach’s choice (print or digital) for a period of at least two (2) years. The Participant agrees that any materials developed or utilized by the Coach for use by the Participant shall be the sole property of Coach. The Participant agrees to preserve in confidence and not to publish, license, sell, distribute or otherwise disclose any such items without written consent of the Coach.

To ensure a safe, open and judgment-free experience that supports comfort with vulnerability and growth, Participants are encouraged to communicate to Coach or the group any additional confidentiality needs that are not explicitly stated herein. Participants may also choose not to share their company name, to anonymize details when sharing, or propose topics confidentially in advance -- as long as there is sufficient detail for group discussion. Moreover, ​​Participants are responsible for securing a private space for sessions and agree not to have anyone else join or listen in on any part of the program.

PAYMENT & CANCELLATION

 

​The payment is due in full to Quilting Health LLC at least 48 hours prior to the start of the session. Full reimbursement of session payment is available if session is cancelled at least 48 hours prior to start. Sessions not cancelled by Participant at least 48 hours in advance will result in forfeiture of payment. Sessions cancelled by Coach will be rescheduled at mutual convenience of the Parties or reimbursed in full. Please contact connect@QuiltingHealth.org or (+1) 857-997-2831 with any questions.

The Participant or Coach each has the right to terminate their engagement at any time with or without cause. Participant agrees to compensate the Coach for amount due under this Agreement through the termination date, including any associated expenses and fees.

LIMITATION OF LIABILITY

 

The Coach makes no guarantees, representations or warranties of any kind with respect to the services agreed upon and rendered. The Coach assumes no responsibility for errors or omissions that may appear in any of the shared materials. The Coach assumes no responsibility and shall not be held liable for Participant choices or actions within or outside of coaching sessions.

 

The Participant agrees to defend, indemnify, and hold harmless the Coach from and against any and all liabilities and expense whatsoever which Coach may incur or become obligated to pay arising out of or resulting from the coaching services or from interactions with other Participants. The Coach’s entire liability under this Agreement, and the Participant's exclusive remedy, shall be limited to the amount paid by the Participant to the Coach under this Agreement for services rendered, including associated expenses and fees.

 

ENTIRE AGREEMENT

 

This document, together with Coach's Privacy Policy and Terms & Conditions, reflects the entire Group Coaching agreement between you and the Coach, and complete understanding of the Parties with respect to the subject matter. The provisions of "Confidentiality" and "Limitation of Liability" shall survive the termination of this Agreement. If any provision of this Agreement shall be held to be invalid or unenforceable for any reason, the remaining provisions shall continue to be valid and enforceable.

Following payment and before start of first group coaching session, you will receive a copy of this agreement and meeting Zoom link at the email address listed in your payment, unless you specify otherwise.
